Participant Waiver & Release of Liability

Participant understands that participation in golf simulation activities involves inherent risks, including but not limited to swinging golf clubs, golf balls, electronic equipment, confined spaces, and interaction with other participants. Participant knowingly and voluntarily assumes all risks of personal injury, property damage, or loss, whether caused by Participant, other participants, or the equipment.

Assumption of Risk

In consideration of being permitted to participate in golf simulator activities provided by Greenies Mobile Golf (“Provider”), the undersigned participant (“Participant”) acknowledges and agrees as follows:

Physical Condition

Participant, on behalf of himself or herself, and his or her heirs, assigns, and representatives, releases, waives, discharges, and holds harmless Greenies Mobile Golf, its owners, officers, employees, contractors, and representatives (“Releasees”) from any and all claims, demands, actions, or causes of action arising out of or related to participation in golf simulator activities, including those caused by the ordinary negligence of the Releasees, to the fullest extent permitted by Ohio law.

Release of Liability

Participant certifies that he or she is in good physical condition and capable of safely participating in the activity. Participant agrees to follow all verbal and posted safety instructions provided by Greenies Mobile Golf staff.

Damage to Equipment

Participant agrees to be financially responsible for any damage to equipment or property caused by misuse, negligence, or failure to follow instructions.

Medical Authorization

Participant authorizes Greenies Mobile Golf to obtain or provide medical assistance deemed necessary in the event of injury. Participant understands that Greenies Mobile Golf does not provide medical insurance and agrees to be responsible for any medical costs incurred.

Alcohol Acknowledgement

If alcohol is present at the event, Participant acknowledges that consumption is voluntary and agrees to participate responsibly. Participant assumes all risks associated with participation while alcohol is present.

Photo & Video Release

Participant grants Greenies Mobile Golf permission to use photos or video taken during the event for marketing and promotional purposes unless otherwise requested in writing prior to the event.

Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of Ohio.

Acknowledgment

By signing below or submitting this form electronically, Participant confirms that they have read, understood, and voluntarily agree to this Waiver and Release of Liability.
